Since ACA has different carrier contracting paperwork, using the request form below you can select the carriers and we will send you the appropriate paperwork for the carriers you selected. Make sure to get your contracts back to us in advance of Open Enrollment.

If you would like to be able to market United Business Association (UBA), Healthy America Association (HAA), or Benefit Boost (BB) plans through HealthyAmerica, complete the simple online contracting. Included in the package is our contract and the appointment paperwork for SiriusPoint, GTL and United States Fire Insurance Company.

COMPLETE & E-SIGN CONTRACTING

BENEFIT BOOST ONLY CONTRACTING

If you do not want to market anything but the non-insurance Benefit Boost Subscription programs, complete this stripped down online contracting package which omits all of the carrier appointment paperwork. You do not have to have a license (resident or non-resident) license to market Benefit Boost since these are non-insurance products. Anyone is eligible to market these plans.
